Orange County: what shapes the choice here

Orange County has one of the most competitive Medicare Advantage markets in the state, with SCAN, Alignment, Blue Shield, UnitedHealthcare, Humana, Wellcare and Kaiser all present and $0-premium plans everywhere. The trade-off is the medical group: pick the plan, then the group, then find out whether your Hoag or UCI specialist is in it.

South County retirees in Laguna Woods, Mission Viejo and San Clemente tend to ask about Providence Mission and Saddleback first; north-county residents about UCI, St. Jude and St. Joseph. A Medigap policy sidesteps all of it, at a premium the birthday rule lets you re-shop each year.

What to watch: Orange County’s Advantage plans are built on medical groups — Hoag Physician Partners, Optum, Monarch, Greater Newport, MemorialCare Medical Group — and the same plan name can mean a different set of doctors from one group to the next. For 2026 Anthem’s PPOs left and Aetna reduced plans here. Dual-eligible residents have CalOptima OneCare, the county’s Medi-Medi Plan.

Counties: Orange.

When you can enroll

Turning 65? Your Initial Enrollment Period is the 7 months around your birthday. Already on Medicare? The Annual Election Period is Oct 15–Dec 7, Medicare Advantage Open Enrollment runs Jan 1–Mar 31, and California’s Medigap birthday rule gives you 60 days from your birthday each year to switch supplements without health questions.

Your options

Which kind of plan fits you?

Medicare Advantage

All-in-one Part C plans, often $0 premium, built on a county network — the choice of about half of California, and the one that changed most for 2026.

How Advantage works

Medicare Supplement

Medigap pairs with Original Medicare and works with any provider nationwide — UCLA, Stanford, Cedars, City of Hope, or your doctor in Arizona — with California’s birthday rule to switch every year.

How Medigap works

Part D drug plans

Standalone drug coverage chosen around your medications and pharmacy. 2026 out-of-pocket cap: $2,100.

How Part D works

Kaiser Permanente

Senior Advantage is its own world: everything in one system, and no Medigap route in. When it fits, and when it doesn’t.

Questions from Orange County

Frequently asked questions

Which Medicare plans are available in Orange County?

A deep Medicare Advantage menu, CalOptima OneCare for people with both Medicare and Medi-Cal, every Medigap carrier in California and the full Part D lineup. Availability changes each plan year.

Does Hoag or UCI Health take my Advantage plan?

Each system contracts with some Medicare Advantage plans and not others, and the lists change every October. With Original Medicare and a Medigap policy there is no network to check, except Kaiser, which is reached only through Kaiser Senior Advantage. We confirm your doctors before you enroll.

How much does your help cost?

Nothing. Independent agents are paid by the carriers when you enroll, so comparing plans and reviewing your coverage each year is free to you.
