Los Angeles County: what shapes the choice here

Los Angeles County is Medicare at its most crowded: dozens of Advantage plans, hundreds of medical groups, and a care map where the plan you pick decides whether you see a UCLA cardiologist or a Cedars one. Advantage plans here work through delegated medical groups (Optum, Regal, HealthCare Partners, Facey, Torrance Memorial IPA and others), so the question is not only which hospital but which group your primary doctor belongs to.

Kaiser is its own answer: roughly a third of the county’s seniors take Medicare through Senior Advantage and never touch the rest of the menu. For everyone else, a Medigap policy with Original Medicare opens every non-Kaiser door in the county at once, and California’s birthday rule lets you re-shop it every year.

What changed for 2026: Anthem Blue Cross ended its Advantage PPO plans in California and Aetna closed plans across the county, which pushed a lot of Angelenos who chose a PPO for its out-of-network freedom into a decision between an HMO, a different PPO and a Medigap policy. The county menu is still the deepest in the country — Kaiser, SCAN, Alignment, Blue Shield, UnitedHealthcare, Humana, Wellcare, Central Health, Brand New Day and many more — and every plan rides on a different set of medical groups.

When you can enroll

Turning 65? Your Initial Enrollment Period is the 7 months around your birthday. Already on Medicare? The Annual Election Period is Oct 15–Dec 7, Medicare Advantage Open Enrollment runs Jan 1–Mar 31, and California’s Medigap birthday rule gives you 60 days from your birthday each year to switch supplements without health questions.

Availability is set by county and changes each plan year; for 2026 Anthem’s PPOs left and Aetna reduced its plans here.

Does UCLA or Cedars-Sinai take my Advantage plan?

Each system contracts with some Medicare Advantage plans and not others, and the lists change every October. With Original Medicare and a Medigap policy there is no network to check, except Kaiser, which is reached only through Kaiser Senior Advantage. We confirm your doctors before you enroll.
